The LAST Medication pick up will be on Friday, April 24th, 2020 from 5-6 pm at the Van Vleck Elementary parking lot (for all grade levels). The Nursing Team will reach out to you individually this week to notify you if your student still has medications at the school.
This will be the final medication pick up for the 2019-2020 school year. If you are unable to make it, anyone on the student's registration or medical authorization form can pick up the medication. If you are still unable to make it, notify me ASAP by filling out this form. If you choose not to pick up the medication, the medication will be properly disposed of per the Texas Department of State Health Services, FDA, & CDC guidelines after May 21st.
Please stay in your vehicle and roll down your passenger window for communication.
This will be a limited contact pick up- you will "verbally" sign off that you received the medication and this will be documented for our records.
